Red Leg 1

This man presented to ER with left leg cellulitis and was given ancef and probenecid. Then he was admitted, abx changed to ceftriaxone. Now 4 days of therapy and leg hasn’t worsened but hasn’t improved either. PMH – type II DM, HTN, bipolar, prostate (?), dyslipidemia. Also hyperkalemia and r/o DKA on admission. Vomiting at triage. Physician in hospital is not regular GP. Not febrile but using PRN acetaminophen. MRP asked me to review antibiotic management.

What would you do?
